MTA Direct Pulp Therapy: When We Can Save a Tooth's Nerve
Sometimes during treatment, we discover that a cavity has reached the deepest part of the tooth and a very small area of the nerve becomes exposed.
Years ago, this situation often meant a child would automatically need a more extensive procedure, such as a pulpotomy. Today, advances in pediatric dentistry allow us to be much more conservative in carefully selected cases.
When the nerve remains healthy and the conditions are right, our pediatric dentists may be able to protect the exposed area and help the tooth continue functioning normally through a procedure called MTA Direct Pulp Therapy.
What Is MTA?
MTA stands for Mineral Trioxide Aggregate.
While the name sounds technical, the concept is simple.
MTA is a highly biocompatible material that is placed directly over a small nerve exposure to help protect the pulp, encourage healing, and preserve the vitality of the tooth.
It has become one of the most trusted materials in modern pediatric dentistry because of its excellent clinical performance and long-term success.
When Is MTA Direct Pulp Therapy Recommended?
This treatment is only recommended in very specific situations.
Our pediatric dentists may consider MTA Direct Pulp Therapy when:
The nerve exposure is very small
The surrounding pulp appears healthy
There are no signs of infection
The tooth is not causing spontaneous pain
X-rays show healthy supporting structures
The tooth can be predictably restored
In these cases, our goal is to preserve the health of the tooth rather than immediately moving to more extensive treatment.
What Does This Mean for My Child?
Think of it this way.
If a cavity is approaching the nerve, we always want to preserve as much healthy tooth structure and healthy pulp tissue as possible.
When conditions are favorable, MTA allows us to place a protective layer directly over the exposed area and give the tooth an opportunity to heal and remain healthy.
For the child, this means we may be able to save the nerve and avoid more invasive treatment.
How Is MTA Applied?
After the cavity is removed and the area is carefully evaluated, a small amount of MTA is placed directly over the exposed pulp tissue.
The tooth is then sealed and restored, typically with a filling or crown depending on the amount of remaining tooth structure.
Once the tooth is protected, we continue to monitor it during routine examinations to ensure it remains healthy.
Why Don't All Offices Use MTA?
This is a question many parents don't know to ask.
MTA is significantly more expensive than many alternative materials used in dentistry. In many cases, insurance companies do not provide additional reimbursement for its use.
As a result, some offices choose less expensive materials when treating deep cavities.
At our office, we make treatment decisions based on what we believe provides the best clinical outcome for the child sitting in our chair.
If we believe MTA offers the greatest opportunity for long-term success, that's what we use.

Many teeth treated with MTA Direct Pulp Therapy continue to function normally and remain healthy for years.

The health of the tooth at the time of treatment
Good oral hygiene
Regular dental visits
Ongoing monitoring
